The Community Directed Diabetes Prevention and Treatment Program provides:

Diabetes Clinic Schedule

Since the late 1990’s, the Toiyabe Indian Health Project-Special Diabetes Program for Indians, has provided a walk-in clinic for Toiyabe Native patients who are on the diabetes registry. The Diabetes clinics are held on the 3rd Thursday of every month from 8:00-11:00 AM, by appointment.

We offer:

A complete physical exam, if needed
Podiatry Exam
Nutrition Consults
Medication Review
Lab Consults
Dental Consults
Optical Consults

Community Health Representatives (CHRs) can transport, based on eligibility. Call 760-872-2622 to learn more. Incentives, raffles, and snacks are also available. It’s ‘one-stop shopping’ for people with diabetes.
