Healthy Heart Program

The Healthy Heart Program is designed to prevent cardiovascular disease complications in Native Americans already diagnosed with diabetes.

This program uses a case management approach to help participants take responsibility for their health. The Healthy Heart Program also helps patients make lifestyle adaptations that lead to increasing exercise and healthy dietary practices while decreasing body weight, cholesterol, blood pressure, and other biological markers associated with the risk for heart attack, stroke, and peripheral vascular disease.

Program participants meet monthly with their case manager, undergo annual physical examinations and lab work, and participate in a diabetes education curriculum.
